Chronic Migraine : Chronic migraine (CM) best characterizes those patients with a history of migraine who experience headache more than half the time.Refractory Migraine (RM) : Despite advances in headache therapies, there remains a group of sufferers with refractory (intractable, treatment - resistant) headache who fail to respond to or cannot tolerate current evidence – based treatments.

Although the concept of Refractory Headache (RH) has a long history, there have been few attempts to formalize an operational definition. The International Classification of Headache Disorders, Second Edition (ICHD - 2) does not include a definition for RH. Certain migraines are labelled as refractory, but the entity lacks a well – accepted operational definition. Efforts are now been made to reach a consensus on refractory migraine which includes – persistent and disabling migraine despite several adequate management trials.

Rebound/Medication Overuse Migraine : Analgesic agents are prescription or over-the-counter medications used to control pain including migraine and other types of headaches. When used on a daily or near daily basis, these analgesics can perpetuate the headache process. They may decrease the intensity of the pain for a few hours; however, they appear to feed into the pain system in such a way that chronic headaches may result.

Cause and Treatment : At present, there is no known etio - patho - physiology (cause) of migraine, though; various theories have been proposed by leading scientists in past decades. Similarly, there is no specific laboratory investigation for the diagnosis of migraine.

However, certain factors are known to trigger it. Common triggers of migraine include genetic and environmental factors, tyramine containing food, sulphites, nitrites and hormones. There are few other factors which might trigger attacks of migraine among individuals differently.

Specialities

Migraine is the most common primary episodic headache disorder. It is derived from Greek word ‘Hemicrania’ which means half of the head; it means a headache which affects half of the head. However, the latest understanding on migraine is as follows: Migraine is one of the varieties of recurrent headache which is not always a strictly unilateral headache; it can be bilateral and is characterized by various combinations of neurological, gastrointestinal and autonomic symptoms.

Signs and Symptoms: Accurate diagnosis of any disease is important because it has specific treatment implications. However, at present, headache diagnosis is clinical; hence there are no “gold standard” tests or biologic markers that exist for the diagnosis of migraine. In the absence of any objective parameter, the most widely used headache classification system was first developed in 1988 by International Headache Society (IHS) and revised in 2004.

Migraine has been defined as a major neurological condition among debility causing diseases by World Health Organisation. Migraine commonly starts at puberty and affects those aged between 30 – 40 years but can trouble much younger and elderly people, including children. As migraine is more troublesome at the productive years, financial cost of society, principality from lost working hours and lost productivity are incalculable.

Thus migraine brings substantial health care burden both clinically and economically to the individuals and their families. However this problem worsens when migraine converts into chronic daily headache in 4% of general population and in 22% of migraine sufferers.
